Primary Care
We provide comprehensive care for a wide range of health issues. We can treat common conditions and assist with the management of chronic health conditions by connecting to your existing provider or linking to a new one.
Social services
We can help with referrals to housing, employment, legal aid, food, child care and other services as needed.
Care coordination
We work with our patients’ doctors, specialists, and case managers to assist with linkage to primary care and specialty services as needed.
